By voluntarily submitting my photograph/drawing/artwork for this project, I hereby certify that the work is an original piece created by me and/or I have permission to make this submission, and grant the City of Thousand Oaks a non-exclusive license for the use of the submitted work, as well as my name, in any and all City publications, including web site entries, without payment or any other consideration, in perpetuity. I understand and agree that the image can be used by the City of Thousand Oaks as stated above and that I may also continue use it for myself personally or professionally. I hereby irrevocably authorize The City of Thousand Oaks to edit, alter, copy, exhibit, publish or distribute this image for purposes of publicizing City of Thousand Oaks programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my image appears as well as any right to royalties or other compensation arising or related to the use of the image. I hereby hold harmless and release and forever discharge The City of Thousand Oaks from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I hereby certify that I am 18 years of age or older and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release. If under the age of 18 I will have consent signed by a parent or guardian.
